Actually, in this instance you do not need to create any macros for the Sky Box. Rather you will install the CableTV driver. Add a new CableTV device. You set the devices IR Device and IR model and put the device into a room. When you save the server will automatically create the activation macro for you. I have new documentation for the CableTV driver going up on the website tomorrow that will document that entire process. If you can wait from the documentation to go up it will be a good test.
However, that does not eliminate the need for a good macros primer.
Is there something similar for a IR controlled Blu-Ray player? Or do I need to write my own macro for it?

I thought we're supposed to use the GenericIR driver? In the documentation you just linked it even says to download the GenericIR driver.
Yes, you use both. The GenericIR to get your hardware IR codecs in the myServer local database, and the BluRay Driver to map myServer AV commands to the IR commands.
I've just tried with a Samsung BluRay player and Sky satellite box (pretending it's cable TV) and neither of them have created an activate-source macro.
Of course it's quite possible I'm not following the required steps exactly - I'm looking forward to the new CableTV driver documentation that should explain everything.

So should it be something similar for an "AVR" that I setup Generic Serial commands for?
Actually I've got basically the same question for my DVR, which I found IR commands for but not "driver" per se.
